A new mRNA vaccine from the new U.S manufacturing company Moderna has been announced by the company that it has got 95% success in its large scale trial. 30,000 volunteers have been given the vaccine. Out of the trial, only 5 people developed mild COVID-19 symptoms were as 90 people have shown symptoms of COVID-19 among those who received placebo shots
Previous to this Pfizer company already announced its mRNA vaccine candidate got the success of 90% in its large scale human trial.

Light of Hope:-Vaccine Trials Take Center Stage
A recent progress of vaccine clinical trials shines the light of hope for this yet unresolved deadly pandemic.
The first human vaccine trial for SARS-CoV-2 virus has been confirmed by THE UNIVERSITY OF OXFORD in April 23.
The vaccine is based on an adenovirus vaccine vector and the novel corona virus spike glycoprotein and has been produced in Oxford.
.The aim is to stimulate the body immunity system,to recognize and to react with the virus's spike protein so as to prevent it to fix and fuse with human cell.
Two scientists acted as volunteers to receive the inoculation last week.5 to 6 months have to be passed to confirm the safety and effectiveness of the vaccine.The total numbers o volunteers they expect are around 1110.
They would be divided into two batches,one would receive the corona vaccine and the other would receive a meningitis vaccine as a control The vaccines are to be given to the volunteers so that they do not which vaccine they received.Up to the last week only two volunteers have come forward for the trial.
The volunteers are :-
1.Edward O' Neill.Ph.D from the Department of Oncology at the University of Oxford.
2.Eliza Granato.Ph.D,from the Dept.of Zoology,University of Oxford.
one of the above was inoculated with the control.
Could Ivermectin Kill the Virus in 2 Days?
A new cell culture study in Australia has found that an anti lice drug is able to eliminate the novel corona virus within two days.However whether this therapy is safe and effective in human beings remains to be seen.
Ivermectin is an anti parasitic medicine which can be used for prevention,treatment and control of many parasites like head lice,scabies,pin worms,and thread worms.

A New Treatment Target Found
Researchers from the Cornell University in Ithaca New York have got important findings to combat with this novel corona virus.
They started their researches by drawing correlations with other corona viruses such as SARS-CoV-1 and MERS-CoV
In their research they mainly concentrated at the structure and the knitted method of the spike proteins on the surface of the virus.
A single spike protein has two parts,namely the S-1 and S-2 proteins.
the S-1 part is the visible part extruded out side the surface which the virus uses to fix with the human cell receptor.
The other S-2 portion is inside the surface and invisible which the virus uses to fuse with the human cell for a cell entry.Hence this S-2 protein has the fusion machinery that helps the virus to break the barrier between the human cell protective layer and the virus body layer in order to inject the viral contents into the human cell.
They found that all the three virus have similar S-2 proteins.
They discovered that calcium ion helps the viral fusion machinery to function.
The team is trying to find an antibody that could inhibit this fusion process by targeting the S-2 protein.
Blocking the fusion step is important as the fusion machinery protein is the one which evolve slow changes when compared with other parts of the virus.

REMDESIVIR WINS FDA'S EUA
Gilead Pharma's anti viral drug Remdesivir scores emergency FDA authentication in the treatment of COVID-19 days after a big positive data reveal.
Days after the U.S officials and others reported the first positive controlled data for this anti viral drug the FDA issued its Emergency Use Authorization.
The FDA very carefully says in its EUA data sheet that there are no FDA approved drugs to treat COVID-19 but remdesivir may benefit some patients in the hospital.
The FDA chief Denise Hinton said in the authorization the EUA is issued because of the deadly nature of the disease and the lack of proper treatments.The EUA is valid only to the patients with severe condition.
Dr.Denise Hinton is cautious to issue the EUA on the basis of available data evidences that it is reasonable to decide at this moment that this drug may be effective in treating COVID-19 when used under the conditions described in the authorization letter that the potential benefits outweigh the risks.
The FDA issued the nod on the basis of the closed,and controlled positive study report from National Institute for Allergy and Infectious Disease (N.I.A.I.D.).In that study they found that remdesivir cut recovery time by 4 days or 31% when compared with placebo for those who have been hospitalized for COVID-19.
Dr.Anthony Fauci of N.I.A.I.D. who studied the drug said that although 31% benefit does not seem like a knock out 100% but it is an important proof of concept because what remdesivir has proven is that a drug can block this virus.
When experts believe the drug would not amount to a cure but a former FDA staff believe that it can be a toolbox for researchers and doctors to be better prepared to deal with the pandemic.
It is important to note that a few days before FDA issued similar E.U.A on chloroquine and hydroxychloquine and later on withdrawn it.

4-aminoquinolines are the drugs used as antimalarials for decades.There are many types in this class out of which two drugs namely chloroquine and hydroxychloroquine are highlighted by the VIPs like U.S.President Donald Trump and the French President Emmanuel Macron and many more as the drugs of choice for the treatment of the Covid-19.

Chloroquine and hydroxychloroquine are having serious side effects from nausea vomiting and headache to serious episodes of retinopathy,liver,kidney and cardiac damages.Rare but serious ventricular arrhythmias,and hypoglycemia can be precipitated leading to coma and death if people under go self medication carelessly with 4-aminoquinoline drugs.
He further asked the FDA and CDC authorities to clear the ambiguity arouse by the authorization.He further question that while we undermine these side effects to some extent if a proven result is achieved but if not who will take the responsibility of putting the public health at risk.
At the end of March '20 the FDA withdrawn its EUA and the CDC has published a statement that 'still there are no drugs has been proved as a cure for Covid-19
Moreover these drugs are used as well proven treatments for malaria and autoimmune diseases like rheumatism and lupus erythematosus.
The question is if these life saving drugs are depleted for the use of unproved covid-19 treatments then would that not be enough to put the patients of malaria and autoimmune diseases at risk?
More over chloroquine and all other 4-aminoquinoline drugs were unsuccessfully tried to treat mononuleosis (H4N1) in 1960 with unsatisfactory results.
Since then chloroquine was tested in laboratories for a range of viruses from influenza(H5N1) to Zica virus.But the results are mixed.Some positive and many negative results.In cells infected by Epstain-Barr virus {(H4N1) which causes mouth ulcers,swollen lymph nodes,elevation of monocytes in the blood,} chloroquine increased virus replication.In one study chloroquine reduced the viral transmissions of Zica virus in the offspring of five infected mice.
In another study chloroquine inhibit viral replications of Ebola in a lab in vitro test but worsen the infection in guinea pigs that is in vivo test.
Similarly chloroquine was actively inhibit the virus replication of chikungunia in the laboratory (in vitro)but worsen the condition in monkeys (in vivo)
In the laboratory tests with SARS Co-2 chloroquine positively inhibit the viral replications,blocking the ACE2 receptors. Generally it produces an unfavourable alkaline pH to prevent the viral entry into the human cell by cell fusion.The virus needs acid pH (pH around 2 to 5) for all its activities.But as weak bases chloroquine and hydroxychloroquine increase the cell pH to an unfavourable alkaline condition and thereby inhibit the viral activities.But these positive results are obtained in laboratories only.
But when given to human body because of other complicated factors the scientist cannot predict any positive results.

Most Patients Inaccurately Self-Report Antibiotic Allergy

they’re taking accurate and appropriate treatment.
A recent study presented at the American Academy of Allergy, Asthma, & Immunology (AACI) 2016 Annual Meeting investigated the extent to which self-reported allergies were actually true. To do so, researchers from the Cleveland Clinic compiled demographic and allergy data for outpatients and inpatients who received an “allergy” point-of-care assessment between August 1988 and August 2015.
The number of patients with self-reported allergies totaled nearly 1.5 million, and 47% of them claimed an allergy to beta-lactams, namely penicillin and amoxicillin. Other top-reported allergens were sulfa, codeine, morphine, seasonal allergies, aspirin, latex, contrast, and iodine.
Ultimately, however, the researchers observed an actual penicillin allergy in just 4.6% of cases analyzed, and those patients were predominantly male and primarily aged between 35 and 56 years. Nevertheless, “the overwhelming majority of self-reported ‘allergy’ involves antibiotic or analgesic medications,” the researchers wrote.
“These data highlight the substantial morbidity related to medications, particularly beta-lactams, withheld due to self-reported ‘allergy’ in a large tertiary care center population,” they concluded.
Patients who believe that they’re allergic to penicillin are typically given alternate antibiotics prior to surgery in order to ward off infection, but those alternatives can be more toxic, more expensive, and less effective.
Notably, previous studies have also shown that many patients who claim to have a penicillin allergy may not actually be allergic to the antibiotic. In fact, literature suggests that only 10% of patients labeled with a penicillin allergy have a true immunoglobulin E -mediated allergic reaction.
In 1 such study, researchers tested the skin of 38 patients at a particular medical center who claimed they were allergic to penicillin in order to determine whether the testing could help reduce the cost of expensive alternative antibiotics. Every patient in the study tested negative for penicillin allergy, which ultimately led the medical center’s staff to alter the medication regimen for 29 patients. Those changes significantly lowered the prescription costs for those patients, the researchers reported.
“When you are told you have an allergy to something, it's important to be seen and tested by an allergist who has the specialized training needed for accurate diagnosis and treatment,” ACAAI President-Elect James Sublett, MD, previously stated. “If you're truly allergic to a medication, your allergist will counsel you on an appropriate substitute.”’
Pharmacists can play a critical role in curbing inaccurate allergy reports by asking patients at the time of medication review whether they’ve sought the opinion of an allergist to confirm their allergy. Health systems should consider implementing guidelines to ensure such medication reconciliation occurs.

ER guaifenesin and pseudoephedrine use with a “wait and see” approach could help decrease patient demand for antibiotics.
Many patients expect antibiotic therapy upon presentation with respiratory symptoms, but antibiotic overuse and misuse (eg, for viral illnesses) drives resistance.
Deferring antibiotic therapy with ER guaifenesin and pseudoephedrine provides symptomatic relief that may reduce patients’ desire for antibiotic therapy.
The researchers enrolled 1179 patients (591 in active arm, 588 in placebo group) with chest congestion, thickened mucus, nasal congestion, runny nose, sinus headache, sinus pressure, and/or postnasal drip.
Fewer patients desired antibiotics after 7 days of active treatment (4.2%) than placebo (8%). Guaifenesin and pseudoephedrine were better tolerated than antibiotics, which tend to cause stomach upset in a large number of patients.
The researchers concluded that treatment of viral respiratory infections with guaifenesin and pseudoephedrine decreased patient demand for antibiotics and is effective and well-tolerated
Flurbiprofen lozenges may help suppress cough.
Because inflammatory mediators drive both upper respiratory tract infection-induced cough and sore throat, a local anti-inflammatory could be an effective treatment.
The researchers enrolled 101 patients in the active arm and 21 patients in the control group. All subjects had sore throat (a score of 5 or greater on the Tonsillo-Pharyngitis Assessment) and at least 1 upper respiratory tract infection symptom.
The patients received a single lozenge, which was either an 8.75 mg flurbiprofen lozenge or a visually identical placebo.
Almost half of the active arm patients with cough (11 of 24) reported absence of cough 3 hours after receiving the lozenge. No patients reported adverse effects.
The researchers concluded that the use of a flurbiprofen 8.75 mg lozenge appeared to have an antitussive effect mediated by a local anti-inflammatory effect.

the health ministry has banned around 350 fixed dose combination (FDC)
drugs that were widely available in the market till now. These medicines
include FDCs based on codine, used in popular cough syrups like
Phensedyl and Corex.
The government has decided to prohibit manufacturing and sale of these medicines because they were found to be "irrational" without any therapeutic efficacy and use. Also, in some cases, there were concerns about misuse of such medicines considered unsafe for mass consumption, official sources told TOI.
While the health ministry has already approved the ban order with immediate effect from Friday, the gazette notification is likely to be printed by Monday, a health ministry official said.
