KIDNEY AND URINARY TRACT INFECTIONS-1


RENAL AND UT- INFECTIONS

   Microbial infection too affects kidneys and can cause either CKD, AKD, or ESKD apart from other factors that have been already described.
The most common type of infection in this area is E.Coli or Escherichia coli which accounts for almost 80% of the kidney and UT infections. The other organisms which have a similar affinity for kidneys and UT are Klebsiella, Staphylococcus saprophyticus, Pseudomonas, and Enterococcus.

In the figure above (Fig-1), it is clearly seen that any infections which start at the urinary tract (urethra) travel upward to the bladder.
All untreated urinary tract infections if allowed to stay for a longer period may spread further upward to affect the kidney via the ureter.

Sources of E.coli:-
Un cleaned, and improperly cooked meat like ground beef, raw milk, contaminated fruits, and raw vegetables.
Consuming vegetables like leafy salads, lettuce, and sprouts may cause e.coli infections.
Drinking uncleaned contaminated water, and swimming in a contaminated pool also cause this infection.
Animal and human feces may contain E.coli.

E-coli is harmless as long as it is a part of our gut bacterial flora which is endogenous. (See Fig.3)
But as an invader from outside these stains become pathogenic to our body
In other words, if the harmless e.coli present in the gut flora passes into the urinary system may become harmful to produce serious UTI and KI(Kidney Infections). But rarely gut bacteria can pass into the urinary system unless if it shed into the feces and pass through the urethral opening. It is safe while passing the stools to cover the genitals.
Ladies are more prone to get UTI and KI if they keep their toilets carelessly and unhygienically.
In ladies, the gap between the anus and the urethral opening is in a line and very short. Hence after using the toilet it is better to wash and wipes out with clean tissue or cloth from front to back.
Sexually active women, pregnant women, and old women are at a greater risk to get UTI.

Signs and Symptoms:-UTI

1.Burning and paining when urinating
2.Pain in the lower abdomen
3.Cloudy or bloody urine.
4.Foul smell in the urine
5. Frequent feeling to urinate.
If the infection reached the kidneys then the following symptoms to be added:-
6.Fever
7.Nausea and vomiting
8.Body pain.
Some older women may not feel much symptoms except frequent feelings to urinate or involuntary leaking of urine 
When the kidney is involved the condition becomes serious and needs immediate medical attention.
Diagnosis & Treatments:-

A thorough urine analysis must be done.
If it contains pus, nitrites like bacterial products, and albumin then the infection should be identified by culture test.
It may take a minimum of 2 to 3 days' time to get the cultural test result.
Before that when the infection is confirmed by the routine test the treatment must be roughly started with any antibiotic sensitive to UTI. But when the organism is identified by the culture test then the antibiotic should be changed to the correct one which is sensitive to the particular organism. (See Fig-4)
